FDA Incident Command System (ICS) Forms

In September 2010, FEMA updated the Incident Command System (ICS) Forms. These forms are intended for use as tools for the creation of the Incident Action Plans (IAPs) and for support and documentation of ICS activities, and for other incident management activities. The updated ICS Forms represent an all-hazards approach and should replace all previously used ICS Forms.  The layout and specific blocks on the forms have been updated, and some forms have been adapted for FDA's particular needs; the functionality of the ICS forms remains the same.

It is recommended that all FDA users familiarize themselves with the updated forms and instructions. Use of standardized ICS Forms is encouraged to promote consistency in the management and documentation of incidents in accordance with the National Incident Management System (NIMS).
